DDS businesses that have a license to operate within Nebraska must renew their licenses annually by May 1.

• a complete and signed "Request for Criminal History Information" form for each officer, director, shareholder, partner or member who is new to that position since the previous May 1

• a check payable to the Nebraska State Patrol in the amount of $15 per each individual for which a Request for Criminal History Information form has been submitted
  
  
• a complete DDS Business License Biographical Questionnaire for each officer, director, shareholder, partner or member who is new to that position since the previous May 1

• a non-refundable application fee in the amount of $150 for the main office location and $100 for each branch office location made payable to the Nebraska Department of Banking and Finance.
